The Post-COVID-19 Interdisciplinary Clinical Care Network (PC-ICCN) is evolving its model of care to best support people living with long COVID.
As part of this work, the regional Post-COVID Recovery Clinics are consolidating into a single virtual clinic serving all of BC. Patients who still require care have been transitioned to the provincial virtual clinic where they can access support from a multidisciplinary health-care team, including nurses, occupational therapists, physiotherapists, social workers, and physicians as required. A nursing case manager will oversee their care and assess their recovery at regular intervals.
What this means for you:
- The provincial virtual clinic is still accepting referrals from physicians for patients who are experiencing symptoms for more than 12 weeks following a presumed COVID infection. The referral form is available online.
- Patients’ symptoms must have been thoroughly investigated and other causes ruled out prior to referral. Please review our list of required diagnostics before making your referral. Referrals that do not meet these requirements will be declined.
- PC-ICCN’s model of care for people living with post-COVID symptoms takes an education-first approach. Patients have access to group education sessions and rehabilitation support from nurses, social workers, physiotherapists, occupational therapists and physicians as required.
- This clinic does not provide diagnostics or facilitate specialist referrals. Managing expectations is important for a successful recovery program. Please ensure your patient understands what supports are offered through our clinic.
