Last chance to apply for one-time grants aimed at developing resources to help family physicians address the social determinants of health (SDH).
The deadline to submit your easy-to-complete online application is Monday, December 17.

Here are three things that you need to know:

  1. There are eight grants of up to $5,000 each available. To apply, fill-out a two-page, online grant application form. We need a brief outline of your objectives, approach, impact and budget.
  2. The funding is flexible. We are looking for applications for proposals for resources or tools that support education, empowerment and advocacy around the social determinants of health (SDH).
  3. This is a unique opportunity to make a difference. Making your idea a reality can help your colleagues from across the province who are looking for ways to address the SDH at the practice/community level to affect change.
