The Public Health Agency of Canada (PHAC) is looking to learn more about the needs of health care providers for professional and patient resources on COVID-19 vaccines, and prioritize their development. In particular, they would like to know what information you would like to learn about the vaccines before receiving it, recommending it, and/or administering it.
The survey should take approximately 10-15 minutes to complete.
