The College of Family Physicians of Canada is calling for cases for upcoming editions of the Innovation in Primary Care series, developed by its Advisory Committee on Family Practice.

The series aims to foster collaboration and idea sharing among Chapters and among family physicians across the country so they can benefit from others’ experiences, knowledge, and best practices.

We are calling for cases on the following two themes to highlight local innovations and approaches to health care that are working well:

Theme 1 – Integration of Pharmacists

The edition for this theme will be a joint publication with the Canadian Pharmacists Association to highlight exemplary instances of interaction and engagement, and shared best practices between family physicians and pharmacists across Canada.

Theme 2 – Mental Health in Primary Care

Case studies should focus on a specific situation, including the problem addressed, strategies used, goals achieved, and lessons learned. Contributor templates for each theme are attached for case submission (in English/French).

 

The deadline for submission is September 14, 2018.
